By purchasing enrollment in the Personal Support Worker (PSW) or Personal Care Aide (PCA) programs (the “Service”) from PeakForm Health and Safety Academy (“PeakForm,” “we,” “us”), you (“the Student,” “the Purchaser”) agree to be bound by the following terms and conditions.
Upon completing registration, you will receive an email containing a link to an online invoice. This invoice must be paid in full via debit or credit card.
1. No Refund Policy
All sales are final. Due to the immediate availability of digital course materials, administrative costs, and the scheduling of instructors for clinical labs, PeakForm maintains a strict No Refund Policy.
Once registration is processed and/or access to the online learning portal is granted, no refunds will be issued under any circumstances. This policy applies regardless of whether the Student completes the full 54-week program, attends the in-person labs, or passes the required assessments.
In the event of a payment dispute or chargeback, PeakForm reserves the right to immediately suspend access to all training materials and withhold certification.
2. Program Requirements & Certification
Registration does not guarantee certification. To receive the Personal Support Worker certificate, the Student must:
- Achieve a passing grade of 70% or higher on all assessments.
- Complete all online theory modules chronologically.
- Attend and successfully demonstrate skills during the mandatory In-Person Lab.
- Complete the required hours for the Working Practicum.
- Ensure all program fees are paid in full.
PeakForm reserves the right to withhold certification if any of these criteria are not met.
3. Practicum & Placement
While PeakForm provides training and support, the Student acknowledges that:
- Practicum Placement: PeakForm will assist in coordinating practicums, but placement is subject to the availability of local care facilities.
- Conduct: Students representing PeakForm during their practicum must adhere to professional standards. Unprofessional behavior may result in immediate termination from the program without refund.
4. Intellectual Property
- All course materials, including videos, quizzes, documents, and presentations, are the exclusive property of PeakForm Health and Safety Academy.
- Materials are provided for your personal, non-commercial use only.
- Sharing login credentials, copying, or distributing course materials to third parties is strictly prohibited and will result in immediate termination of your account.
5. Limitation of Liability
PeakForm is an educational training provider. We are not responsible for:
- Any injury, loss, or damage sustained by the Student during the in-person labs or working practicum.
- Employment outcomes. While our course is certified by NBHSA and recognized by the New Brunswick government, PeakForm does not guarantee employment upon completion.
6. Governing Law
These Terms shall be governed by and construed in accordance with the laws of the Province of New Brunswick, Canada. Any disputes arising from these terms shall be resolved in the courts of New Brunswick.
